Prior to your first appointment, please complete these three in-take forms and bring with you to our first session. It’ll help us get started more efficiently and allow more time to work on feeling better.
Drink plenty of water before your visit (good advice no matter what!)
You may also want to create a list of:
- your top 1-3 goals for what you would like to see be different as a result of our work together
- any medications you are on (if unable to include all on the intake form)
- the traumas (BIG or small) you believe may be connected to the reason for the blocks in obtaining your current goals
